Etiqa Dental Coverage: Accredited Clinics, LOG & Hotline

Etiqa dental coverage depends on how you got your Etiqa card. If your employer enrolled you in Etiqa's Group Medical Benefits, your dental benefits are whatever your company's contract says, which can be anything from none at all to a full dental package. If you bought the individual MyLife+ plan online, dental care is excluded unless it repairs damage from an accident.
To find a dental clinic that takes Etiqa, check Etiqa's online provider lookup, then call the clinic and Etiqa's 24/7 Medical Information Center to confirm before you go. Below are the steps, the phone numbers from Etiqa's own pages, and where Etiqa sits next to the more familiar HMOs.
Is Etiqa an HMO?
Not technically. Etiqa Philippines, formerly AsianLife & General Assurance Corporation, describes itself as a composite insurance company majority-owned by Malaysia's Maybank group. Its health product for companies is called Group Medical Benefits. It says it has over 1,600 accredited hospitals and clinics, around 30,000 doctors and a 24/7 in-house call center.
For a patient, though, it works much like an HMO. You show an Etiqa ID at an accredited provider, Etiqa approves the service and pays the provider directly up to your benefit limit, and you settle anything outside your plan. Etiqa calls its approval document a Letter of Guarantee (LOG). It's the same idea as the LOA at Maxicare or Intellicare.
Does Etiqa Cover Dental?
It depends on which Etiqa product you have. Here's what we could confirm from Etiqa's published pages:
| Etiqa product | Who has it | Dental coverage |
|---|---|---|
| Group Medical Benefits | Employees (and often dependents) of companies that signed up with Etiqa | Set by your employer's contract. Some groups add dental, some don't. Check your benefit schedule or ask HR. |
| MyLife+ Plan (bought online) | Individuals aged 0β70, one-year coverage | Dental is listed as an exclusion unless treatment is needed to repair an accidental injury. Routine cleaning and pasta are not covered. |
| SME plans sold by brokers | Small businesses buying through an insurance broker | At least one broker sells a "Dental HMO" add-on from a separate dental network. Ask the broker who runs the dental side. |
Etiqa doesn't publish one standard dental benefit list for group members. So if your company plan does include dental, the only reliable source is your own policy documents. Look for a page called "dental benefits" or a dental rider in your benefit schedule.
What dental plans in the Philippines usually include
Where dental is part of a Philippine HMO-style plan, the usual inclusions are:
- Consultation and oral examination
- Oral prophylaxis (cleaning), usually once a year
- Simple tooth extraction (bunot)
- Temporary or simple fillings
- Emergency relief for dental pain
The usual exclusions are orthodontics (braces, aligners), prosthodontics (dentures, crowns, bridges, implants), cosmetic work like whitening and veneers, and most root canals and surgical extractions. That pattern is common across Philippine HMOs, but it isn't a published Etiqa rule. Your benefit schedule is the final word. Our dental HMO guide covers the typical package in more detail.
Which Dental Clinics Accept Etiqa?
Start with Etiqa's own lookup tools. There are two:
- Hospitals and Clinics lets you filter by type (hospital or clinic), province and city.
- Doctors (etiqa.com.ph/doctors.aspx) lets you filter by specialty and by hospital.
Be warned: neither tool is built for dental. When we checked in October 2026, the doctor search had no dental specialty in its list. Only a handful of provider names contained the word "dental", among them Health Partners-Dental Access Inc., Healthworks Medical Dental Center and Diagnostic Laboratory, and Healthlock Medical Dental Diagnostic Services in Pasig. A clinic can be Etiqa-accredited without "dental" in its name, so the lookup is a starting point, not a complete answer.
The most reliable route is to call the Medical Information Center (details below), give your city and plan, and ask which dental providers your plan can use.
Clinics on NgipenHub that list Etiqa
Some clinics in our directory list Etiqa among the HMOs and health cards they accept. These are what the clinics say about themselves, so confirm before visiting:
- Smiles R' Us Dental Centre, Las PiΓ±as: lists Etiqa among the HMOs it accepts, along with Kaiser. Confirm before visiting.
- Scala Dental Clinic, Henson St., Angeles City, Pampanga: lists Etiqa among the HMOs it accepts, along with iCare, Pacific Cross, Filipino Doctors and HPPI. It lists hours of 9AMβ6PM daily. Confirm before visiting.
Accreditation lists change at every contract renewal, on both the clinic's side and Etiqa's. A clinic that took Etiqa last year may not take it now, so make the phone call.
How to Use Your Etiqa Card at the Dentist
Etiqa's availment guide covers outpatient care in general, not dental specifically, but the flow is the same:
- Confirm dental is in your plan. Check your benefit schedule or ask HR. If dental isn't there, the card won't help at the dentist (except possibly for accident-related repair).
- Find an accredited provider and call ahead. Ask the clinic if they accept Etiqa for dental, not just for medical, and which services they'll do under the card that day.
- Bring your Etiqa ID plus a valid government ID. Etiqa says the membership card can be physical or virtual through the SmilePH app.
- Get the LOG before treatment. Depending on where you are, the LOG comes through the provider's coordinator, through the Medical Information Center, or at an Etiqa clinic. For outpatient approvals, Etiqa's FAQ says you can email the doctor's request to the MIC or call the hotline.
- Ask for the covered and not-covered split in writing. If the dentist finds work beyond your plan, like a permanent filling or a crown, you pay for it yourself. Get a quote before the work starts.
- Settle any excess before you leave. Anything over your benefit limit, or anything your plan excludes, is paid at the clinic.
Our step-by-step guide on how to use HMO dental benefits covers the questions to ask at the front desk. They apply to any card.

If You Paid Out of Pocket or Were Charged Extra
Two parts of Etiqa's FAQ matter here:
- Reimbursement. If you paid a non-accredited provider, Etiqa says the claim may be reimbursed, depending on your master policy and remaining benefit limit. Get the outpatient claim form from HR or Etiqa's Downloads page and submit it through HR. Etiqa says processing takes about 30 days after complete documents. For dental, this only helps if your plan covers dental in the first place, so keep the official receipt and the dentist's itemized statement.
- Extra fees at accredited providers. If an accredited doctor asks you for an additional professional fee on a covered service, Etiqa tells members not to pay it and to report it to [email protected]. The same address takes recommendations if you want your own dentist added to the network.
How Etiqa Compares to Other HMOs for Dental
You usually don't pick Etiqa yourself. Your employer did. But if you're comparing job offers or deciding whether to add your own dental plan, these are the differences that matter:
- Group-first. Etiqa's health business is built around corporate clients. Its one individual health plan, MyLife+, excludes routine dental. Some HMOs sell individual prepaid cards with dental already inside.
- Dental is a contract item, not a given. With any group HMO, two employees at different companies can have very different dental benefits. With Etiqa that's especially true, because it publishes no standard dental schedule.
- The lookup isn't built for dental. Etiqa's public provider search has no dental filter, so finding a dentist usually means calling the hotline. Before you compare networks, ask each HMO how many accredited dentists it has in your city.
- Medical network first. Etiqa's published numbers (1,600+ hospitals and clinics, about 30,000 doctors) describe its medical network. It doesn't publish a dental clinic count.
For a side-by-side of the bigger dental HMOs, see our Maxicare vs MediCard vs Intellicare dental comparison.
If your Etiqa plan has no dental
You have three realistic options:
- Pay per visit. For one cleaning a year and the occasional pasta, paying cash is often cheaper than buying a separate plan. Our tooth filling price guide shows typical rates.
- Buy a prepaid dental plan. These are standalone dental cards with a fixed list of services. Compare them in our prepaid dental plans guide.
- Ask HR at renewal time. Dental riders get added when enough employees ask for them.
Members 60 and above: your RA 9994 senior citizen discount (20% plus VAT exemption) applies to dental services you pay for yourself, including anything your Etiqa plan excludes.
Frequently Asked Questions
Does Etiqa cover dental cleaning?
Only if your plan includes dental. Group Medical Benefits plans vary by employer, so check your benefit schedule or ask HR. The individual MyLife+ plan excludes routine dental care like cleaning.
Is Etiqa an HMO or an insurance company?
Etiqa Philippines describes itself as a composite insurance company, formerly AsianLife & General Assurance. Its Group Medical Benefits work like an HMO: you show your card at accredited providers and Etiqa pays them directly through a Letter of Guarantee.
How do I get a Letter of Guarantee (LOG) from Etiqa?
Through the accredited provider's coordinator, at an Etiqa clinic, by emailing the doctor's request to [email protected], or by calling the 24/7 Medical Information Center at (02) 8895-3308. There's also an online request link on Etiqa's contact page.
Does Etiqa MyLife+ cover tooth extraction?
Not for routine extractions. MyLife+ lists dental care as an exclusion except treatment needed to repair damage from an accidental injury.
Can I use my Etiqa card at any dental clinic?
No, only at accredited providers, and only if your plan includes dental. If you go to a non-accredited dentist, Etiqa says the claim may be reimbursed subject to your policy, so keep your official receipt and file through HR.
What if I lost my Etiqa card?
Etiqa says the membership card can be virtual through the SmilePH app. For non-emergency care without your card, its FAQ says to call the hotline before the consultation.
